اثربخشی مصاحبه انگیزشی بر تبعیت از درمان و کیفیت زندگی در افراد مبتلا به پرخوری دارای افسردگی

چکیده

امروزه اختلال پرخوری عصبی یکی از معضلات بهداشت و سلامت و عامل خطر برای بسیاری از بیماری‌­ها است. پرخوری عصبی نه‌تنها ازنظر سلامتی و تأثیرات منفی آن در بعد جسمی اهمیت دارد بلکه از لحاظ روانی، اقتصادی و اجتماعی و صرف هزینه‌های پزشکی نیز قابل‌توجه است. هدف پژوهش حاضر بررسی اثربخشی مصاحبه­ انگیزشی بر تبعیت از درمان و کیفیت زندگی در افراد مبتلابه پرخوری دارای افسردگی بود. روش پژوهش از نوع نیمه­‌تجربی است که در آن از طرح پیش‌­آزمون، پس‌­آزمون و پیگیری استفاده شد. جامعه­ آماری شامل کلیه افراد مراجعه‌کننده به بخش تغذیه و رژیم‌­درمانی کلینیک  بعثت شهر رشت، در سال 1400 بود که 20 نفر شرکت‌کننده بر اساس تشخیص متخصص تغذیه و مصاحبه­ تشخیصی مبتنی DSM-5 برای اختلالات خوردن به‌صورت هدفمند انتخاب شدند. شرکت‌کننده­‌ها به مدت 6 جلسه درمان مصاحبه­ انگیزشی دریافت نمودند. برای جمع‌آوری داده‌ها از  مقیاس رفتار خوردن داچ (ون استرین، فریجترز،  برگرز و دفارس، 1996)، افسردگی (بک، 1996)، تبعیت درمان (مدانلو، 1392) و مقیاس کیفیت زندگی سازمان جهانی بهداشت (1996) استفاده شد. داده‌­های پژوهش  با استفاده از تحلیل واریانس با اندازه‌­گیری مکرر تحلیل شدند. نتایج نشان داد که مداخله مصاحبه انگیزشی موجب افزایش مؤلفه‌­­ها می‌­شود به طوری‌که بین نمرات پیش‌­آزمون، پس‌­آزمون و پیگیری در تبعیت از درمان و کیفیت زندگی تفاوت معناداری وجود داشت که این تغییر در مرحله پس‌آزمون از پیش‌آزمون و پیگیری بیشتر بود (0/005>p). بنابراین با توجه به تأثیر مصاحبه انگیزشی از این روش می‌توان به‌عنوان یک مداخله درمانی جایگزین و مکمل در کنار سایر روش­‌ها استفاده کرد. یافته‌­ها نشان دادند مصاحبه انگیزشی منجر به افزایش تبعیت از درمان و بهبود کیفیت زندگی در افراد تحت درمان خواهد شد.

Nowadays, binge eating disorder is a significant health issue and a risk factor for many diseases. It is important not only for its negative effects on physical health but also for its psychological, economic, social, and medical expenses. This study aimed to investigate the effectiveness of motivational interviewing on adherence to treatment and quality of life in individuals with binge eating disorder and depression. The research method was semi-experimental, utilizing a pre-test, post-test, and follow-up design. The statistical population included all individuals who referred to the nutrition and diet therapy department of Ba'ath Clinic in Rasht in 2021. Twenty participants were selected based on the diagnosis of a nutritionist and a DSM-5 diagnostic interview for eating disorders. They received six sessions of motivational interviewing therapy. Data were collected using the Dutch Eating Behavior Scale (Van Strein, Frejters, Burgers, and Defares, 1996), Beck Depression Inventory (1996), Treatment Adherence Scale (Madanello, 2013), and WHO Quality of Life Scale (1996). The research data were analyzed using variance analysis with repeated measures. Results showed that motivational interviewing significantly improved the components, indicating a significant difference between pre-test, post-test, and follow-up scores in adherence to treatment and quality of life, with greater improvements in the post-test stage compared to pre-test and follow-up (P<0.005). Therefore, motivational interviewing can be considered an effective complementary therapeutic intervention alongside other methods. The findings showed that motivational interviewing will increase compliance with treatment and improve the quality of life in treated people.
