مقایسه اثربخشی آموزش ذهن آگاهی و خودمراقبتی بر عزت‌نفس جنسی در زنان مبتلا به سرطان پستان

چکیده

پژوهش حاضر با هدف مقایسه اثربخشی آموزش ذهن‌آگاهی و خودمراقبتی بر عزت‌نفس جنسی در زنان مبتلا به سرطان پستان انجام شد. تحقیق حاضر طرح نیمه‌آزمایشی از نوع پیش‌آزمون- پس‌آزمون با گروه کنترل و همراه با پیگیری دو ماهه بود و از منظر هدف، تحقیق کاربردی بود. جامعه آماری این پژوهش شامل کلیه بیماران 50-30 ساله مبتلا به سرطان پستان در سال 1400 بودند که در بیمارستان شهید فیروزگر تهران در حال درمان بودند. نمونه پژوهش به صورت نمونه‌گیری هدفمند انتخاب و به صورت تصادفی در سه گروه اختصاص داده شدند. حجم نمونه شامل 45 نفر (برای هر گروه 15 نفر؛ یعنی 15 نفر در گروه آزمایشی 1، یعنی 15 نفر در گروه آزمایشی 2 و 15 نفر نیز در گروه کنترل) بود. ابزار پژوهش شامل پرسشنامه عزت‌نفس جنسی زینا و اسچوارز (1996) بود و گروه‌های آزمایش جلسات آموزش ذهن‌آگاهی (کابات زین، 2015) و آموزش خودمراقبتی کلارک و همکاران (2014) را دریافت کردند. به منظور تحلیل آماری داده‌ها، از تحلیل واریانس آمیخته استفاده شد. یافته‌ها نشان داد که دو شیوه آموزش ذهن‌آگاهی و خودمراقبتی، بر عزت‌نفس جنسی در پس‌آزمون اثر معنی‌دار دارند. بین نمرات عزت‌نفس جنسی در سه مرحله پیش‌آزمون، پس‌آزمون و پیگیری صرف‌نظر از نوع مداخله، تفاوت وجود داشت (0/001>p). از بین دو شیوه آموزش ذهن‌آگاهی و خودمراقبتی، آموزش ذهن‌آگاهی در تقویت عزت‌نفس جنسی زنان مبتلا به سرطان پستان موثرتر بود. لذا می‌توان نتیجه‌گیری کرد که با استفاده از شیوه‌های آموزش ذهن‌آگاهی و خودمراقبتی (به ویژه آموزش ذهن‌آگاهی)، می‌توان عزت‌نفس جنسی را در زنان مبتلا به سرطان پستان بهبود بخشید.

چکیده [English]

The aim of this study was to compare the efficacy of mindfulness training and self-care training on sexual self-esteem in women with breast cancer. The research method was quasi-experimental with a pre-test, post-test, two-month follow-up, and a control group. The statistical population consisted of all patients (aged 30–50) suffering from breast cancer who applied to the "Firoozgar Hospital" cancer research center located in Tehran City in 2022 and underwent mastectomy surgery. The sampling method was purposive. Forty-five available patients were selected (15 in each experimental group) and randomly assigned to experimental group 1 (mindfulness training) and experimental group 2 (self-care training). Fifteen other patients were placed in the control group and did not receive any training. The Sexual Self-Esteem Scale (Zeanah & Schwarz, 1996) was administered in the pre-test, post-test, and follow-up. The training packages were the mindfulness training package (Kabat-Zinn, 2015) and the self-care training package (Clark et al., 2014). Both trainings were applied to the experimental groups (45 minutes each session) weekly, and the control group did not receive any training. The data were analyzed by mixed analysis of variance. Results showed that both mindfulness training and self-care training were equally effective in increasing sexual self-esteem in women with breast cancer (p < 0.001). The two-month follow-up confirmed these results. Using mindfulness and self-care training (especially mindfulness) can increase sexual self-esteem in women with breast cancer. Therefore, mindfulness and self-care training (especially mindfulness) are effective in raising sexual self-esteem, and it is necessary to use these methods to improve the therapy of these women with breast cancer, as these components effectively enhance the health of these patients.
